There was a lot of potential in the early lore that either wasn’t explored or which changed direction. Here are some of the top of my head which waiting for a cup of tea to brew at work.
The Mentor Legion as elite marines slightly similar to the Death Watch and/or Adeptus Custodes who can also lead other Imperial squads
Greater focus in general on mixed Imperial armies
Beastmen in the Imperial Guard
Grey Knights being a non-psychic chapter who all wear terminator armour
Crangor’s Buccaneers as pirates outside the Imperium consisting of humans, eldar and squats
Eldar and orks pirates could have been developed further too
Chaos Renegades being varied disparate Chaos forces without being dominated by marines (or necessarily containing marines at all) including custodes, navigators, tyranids, zoats, etc
Do something with traitor guard
Develop the Chaos squats who created and maintained chaos titans, daemon engines ans chaos androids
Develop the multiple Chaos eldar options that were mentioned
Expanding the multiple Chaos ork options
Blood Axes consisting of mixed ork and human forces
Exodites being equivalent to Imperial Knights (who were basically cosplaying Exodites)
Genestealer cults being associated with Chaos rather than tyranids
Xenos genestealer cults
Zoats existing as hive fleet foot soldiers, leaders of infiltration cults and as independent mercenaries
Tyranid mind slave armies
Tyranid grabber-slasher infiltrators who rapidly change shape using stolen DNA
Vampire covens
Enslavers & other non-daemon warp entities
Space slann
Half-eldar space marines
