# What Pathology, Microbiology and Pharmacology Enable an MBBS Student to Understand and Do ## Introduction Pathology, Microbiology and Pharmacology connect basic biomedical science with clinical care. - Pathology explains the mechanisms and consequences of disease. - Microbiology identifies infectious causes and guides their diagnosis, prevention and control. - Pharmacology explains how treatment should be selected, prescribed, monitored and modified. Together, these disciplines support a continuous clinical process: > Patient presentation → disease mechanism → probable cause → appropriate investigation → diagnosis → treatment → monitoring → prevention Their purpose is not the accumulation of disconnected facts. Their purpose is to develop mechanistic reasoning, diagnostic interpretation, therapeutic judgment, practical competence and patient safety. --- ## 1. Pathology: Understanding Disease ### Central purpose Pathology explains how normal cells, tissues and organs become abnormal, how these abnormalities alter function, and how the resulting changes produce clinical manifestations, investigation findings, complications and outcomes. Pathology helps the student answer four questions: 1. What disease process is occurring? 2. Why did the disease develop? 3. What structural and functional changes should be expected? 4. What do those changes imply for diagnosis, prognosis and complications? ### 1.1 Disease mechanisms Students learn the progression: > Cause → cellular or tissue injury → biological response → structural change → functional disturbance → clinical manifestation → complication The foundational processes include: - cellular adaptation and injury; - reversible and irreversible injury; - necrosis and apoptosis; - acute and chronic inflammation; - healing and repair; - oedema and vascular congestion; - thrombosis and embolism; - infarction and shock; - immune-mediated injury; - genetic and environmental disease; - neoplasia. This transforms learning from naming a disease into explaining its development and progression. ### 1.2 Morphology and disease expression Pathology trains students to interpret: - gross organ changes; - microscopic architecture; - cytological abnormalities; - blood-cell morphology; - bone-marrow findings; - inflammation, necrosis and fibrosis; - tumour differentiation, invasion and metastasis. The central interpretive sequence is: > Morphological finding → underlying mechanism → probable diagnosis → likely clinical behaviour This is fundamental to histopathology, cytology, haematology and clinicopathological reasoning. ### 1.3 Laboratory interpretation Students learn to interpret: - complete blood counts; - peripheral blood smears; - reticulocyte counts; - coagulation studies; - bone-marrow findings; - urine examination; - cytology and histopathology reports; - tumour markers; - organ-function abnormalities. A laboratory result must be interpreted through: 1. the clinical context; 2. the underlying disease mechanism; 3. associated findings; 4. relevant differential diagnoses; 5. possible pre-analytical and analytical errors. The objective is to determine what the result means for the patient, not merely whether the value is high or low. ### 1.4 Differential diagnosis Pathology develops the ability to distinguish diseases with similar presentations. Students learn to differentiate: - reduced production, blood loss and increased destruction as causes of anaemia; - pre-hepatic, hepatic and post-hepatic jaundice; - glomerular, tubular, interstitial and vascular renal injury; - inflammatory, vascular, degenerative and neoplastic processes; - benign and malignant lesions; - primary and metastatic tumours.
