Maybe that could be why it worked for you, since it was your first time. I had submitted the form back in April and June, and had some issues. My first distribution was sent in July and my bank sent it back because, I stupidly told them it's from crypto. Since months I've been trying to fill the form and contacting Celsius to send the distribution again. The submit button just doesn't work anymore for me. Frustrating as hell.
