📘 Ovarian Cancer FIGO Staging at a Glance, by eMRCOG One picture, every stage. Save it, share it with your study group, and test yourself before the exam. Ovarian, fallopian tube and primary peritoneal cancer share ONE staging system (FIGO 2014), and it is a surgical staging. Six boundaries decide almost every exam question: 🔹 Capsule: torn by the surgeon = IC1. Ruptured before surgery or tumour on the surface = IC2. Malignant cells in ascites or washings = IC3. 🔹 Pelvic brim: disease below the brim is stage II, above the brim is stage III. 🔹 Nodes: positive retroperitoneal nodes only = IIIA1. Inguinal nodes = IVB. 🔹 2 cm: microscopic disease above the brim = IIIA2, visible deposits up to 2 cm = IIIB, more than 2 cm = IIIC. 🔹 Organs: liver or spleen CAPSULE = IIIC. Liver or spleen PARENCHYMA = IVB. 🔹 Pleura: an effusion counts only with positive cytology = IVA. 💡 Exam trap: an omental deposit that looked normal at surgery but was positive on histology is IIIA2, not IIIB. 🧠 Quick test: a woman has a 4 cm omental cake and a nodule on the liver capsule, with a normal liver parenchyma. What is the stage? Write your answer in the comments 👇 All illustrations are original eMRCOG artwork. 📘 Vulval Cancer FIGO Staging at a Glance One picture, every stage. Save it, share it with your study group, and test yourself before the exam. Vulval cancer is staged with the FIGO 2021 system, and it changed a lot from the old version. These boundaries decide almost every exam question: 🔹 2 cm and 1 mm: IA only when BOTH are small (size 2 cm or less AND invasion 1 mm or less). If either is larger, it is IB. 🔹 Depth of invasion is measured from the base of the nearest normal rete ridge to the deepest point of the tumour. 🔹 Lower third: spread to the lower third of the urethra, vagina or anus with negative nodes = stage II. 🔹 Above the lower third: upper urethra or vagina, bladder mucosa or rectal mucosa = IIIA. 🔹 Groin nodes: deposit 5 mm or less = IIIA, more than 5 mm = IIIB, extracapsular spread = IIIC. 🔹 Fixed: tumour fixed to pelvic bone, or fixed or ulcerated groin nodes = IVA. 🔹 Pelvic nodes count as DISTANT metastasis = IVB. 💡 Exam trap: in FIGO 2021, spread to the bladder or rectal mucosa is now IIIA, not IVA. Only fixation to bone or fixed or ulcerated nodes make it IVA. 🧠 Quick test: a 1.5 cm vulval tumour invades 3 mm into the stroma. Nodes are negative. What is the stage? Write your answer in the comments 👇 All illustrations are original eMRCOG artwork. 🩺 Itching in Pregnancy? Think Beyond a Simple Skin Problem! 🤰⚠️ Intrahepatic Cholestasis of Pregnancy (ICP) is an important topic for MRCOG Part 2 & Part 3 candidates. The real challenge is not just making the diagnosis, but knowing when to monitor, when to treat, and when to deliver! 👀 💡 High-Yield RCOG Exam Pearls 🔹 Diagnosis: Pruritus without a primary skin rash + peak bile acids ≥19 µmol/L. 🔹 Important Trap: LFTs may be normal despite raised bile acids. If itching persists with initially normal results, repeat bile acids and LFTs. 🔹 Mild ICP (19–39 µmol/L): Consider planned birth by 40 weeks or expectant management if no additional risk factors. 🔹 Moderate ICP (40–99 µmol/L): Consider planned birth at 38–39 weeks. 🔹 Severe ICP (≥100 µmol/L): Increased risk of stillbirth. Consider planned birth at 35–36 weeks. 🚨 🔹 Ursodeoxycholic Acid (UDCA): May provide a small improvement in maternal itching, but does NOT prevent stillbirth. 🔹 CTG & Ultrasound: Normal findings do not reliably predict or prevent stillbirth in ICP. 🔹 Postnatal Follow-up: Confirm resolution of itching and normalisation of bile acids and LFTs at least 4 weeks after delivery. 🎯 THE GOLDEN EXAM RULE: Peak bile acid concentration, NOT ALT or AST, is the key marker for fetal risk stratification and timing of birth. 📌 Timing recommendations are for uncomplicated singleton pregnancies. Individualise care when additional risk factors are present. 🚨 MRCOG Part3 case challenge ⁉️ PATIENT IS UNCONSCIOUS, BLEEDING… AND UNABLE TO GIVE CONSENT 🚑 📝Leah Turner, 23, is brought to the emergency department after being found unconscious near a nightclub at 03:00. She has vaginal bleeding and is now drowsy and confused. No one is with her. You are the ST5 on call. 👩‍💼👨‍💼The examiner asks: “How would you manage her?” This station explores your clinical reasoning, communication and ability to protect a vulnerable patient’s rights. 🗣️ HOW TO STRUCTURE YOUR ANSWERS Use this approach: 🚦PRIORITY → ACTION → REASON → REVIEW Explain what you would do first (Safety!) why it matters and how the next findings would change your plan(Applied Clinical Knowledge!) 1️⃣ DIFFERENTIAL DIAGNOSIS Think about both problems: reduced consciousness AND bleeding. Organise your differentials and explain which urgent causes you need to exclude. Avoid assuming intoxication explains everything. 2️⃣ EXAMINATION AND INVESTIGATIONS Start with immediate assessment and stabilisation. Then describe a focused examination and investigations, explaining what each will help you identify. 3️⃣ SUSPECTED SEXUAL ASSAULT Separate urgent clinical care from forensic assessment. Address capacity, consent, specialist advice and preservation of evidence. Explain how you would protect her dignity throughout. 4️⃣ THE POLICE REQUEST RESULTS Recognise the confidentiality issue. Explain how you would establish whether disclosure is justified, seek appropriate advice and document your decision. 5️⃣ WHEN SHE REGAINS CAPACITY Explain what happened and what care she received. Explore her concerns, discuss her choices and offer appropriate treatment and support without pressure or assumptions. 6️⃣ DOCUMENTATION AND FOLLOW-UP Describe what you would record, how you would assess safeguarding concerns and how you would arrange a safe discharge and follow-up. 💡 EXAM TECHNIQUE Answer the question asked, respond to each new finding and make your reasoning clear. If y… Read in full →
